Home Employers Registration overview Rates and classifications CHOICES CHOICES is a rebate program that rewards you for providing health and safety and return-to-work training courses to your employees. You can apply for the rebate program by filling out the CHOICES Enrollment form . Any training that increases your skills to help you perform your job more safely may qualify for a rebate. Rebates range from 4% to 10% of your annual assessment, up to $25,000 per year. COR- and SECOR-certified businesses automatically qualify for a 10% rebate. To find out what your rebate might be, refer to the CHOICES Incentive Program policy. Apply for a CHOICES rebate To get your CHOICES rebate, complete the CHOICES Enrollment form . You can also apply for the CHOICES rebate by completing Section B of your annual Employer's Payroll Report (EPR) form . You do not need to provide training documentation with your EPR report, but we may ask for proof of training before processing your rebate or as part of our verification and audit processes.
